Sports
All athletes must have a current physical examination on file in the Health Office (within one year of the start date of the season). Free Sport Physicals are offered to students in the spring of each year for qualification for the following school year. Students choosing not to have a school physical are responsible for making sure that physical forms, completed by their doctors, are turned in to the Health Office by the due dates below.
In addition, prior to the beginning of each sport season, an Interval Health History Form must be completed by the parent/guardian to determine eligibility for that particular season. If an injury has been sustained, or if a surgical procedure has been performed, or there is question regarding a medical condition, the student may be required to get approval, in writing, from the family and/or school physician before the student is able to report to practice or tryouts.
In order to begin tryouts/practice with the rest of the team, completed forms MUST be returned to the Health Office by:

For Fall Sports, the nurse will be in the building on the following dates:
- Monday, August 9
- Thursday, August 12
- Monday, August 16
- Monday, August 23
All paperwork must be in by 8 a.m. that morning in order to be cleared for the following date.
For Winter and Spring Sports, forms turned in late will follow these rules:
• If turned in prior to the end of homeroom and medically cleared, may participate the FOLLOWING day. These students will be eligible 24 hours after the form is reviewed.
• If turned in after the end of homeroom, but prior to the end of the school day, it will be treated as though they were turned in the next day, meaning eligibility will be 24 hours from the next day.
Physical/Health Appraisal Form
This form is utilized for new students and annual sport physical.
Interval Health History Form (now available for Fall 2010)
All athletes must have a current physical on file in the Health Office. In addition, the Interval Health History Form inCurrents must be completed. In order to begin tryouts/practice, completed forms must be returned to the Health Office by TBA.
Injury Clearance Form
The parent may sign this release form only if no medical evaluation was obtained. If the parent signs this release form, it must be understood that the school is no longer responsible for this injury.
ONCE A STUDENT HAS GONE TO A DOCTOR, HAS HAD X-RAYS TAKEN,
- OR - WAS TAKEN TO A HOSPITAL, THE PARENTS CANNOT SIGN THIS RELEASE FORM. Once a student seeks medical evaluation, he/she CANNOT return to participation until he/she is released by his/her doctor.

Policy regarding Medication in School
- All medication, including non-prescription drugs such as Tylenol, Motrin, etc. given in school must be prescribed by a licensed medical doctor.
- A written request from the physician must be on file. This request must indicate the dosage and frequency of the prescribed drug.
- A written request from the parent to administer medication must be on file.
The parent must assume responsibility to have the medication delivered to the Health Office in a properly labeled original container.
Medication Form
This form is utilized for all medication, including non-prescription drugs, given in school.
- A written request from the physician must be on file. This request must indicate the dosage and frequency of the prescribed drug.
- A written request from the parent for the school nurse to administer medication must be on file.
- The parent must assume responsibility to have the medication delivered to the Health Office in the original container with the proper pharmacy label.
- The bottom portion of the form must be completed by the prescriber and parent/guardian if the student will be carrying this medication on his/her person.
Health and Emergency Update Form
This is completed with registration and at any time that you need to update the school regarding health concerns and/or new emergency contact information

The first form is the instructions. In the instructions are the Part 1- Application and the part 3- Physical fitness Certification.
Wellness Policy
New Student Physicals
New students to SMHS are required to have a physical exam on file in the Health Office. This applies to all ninth graders and transfer students. Most have had a sport physical and this counts for your physical exam. If you have not turned in a physical form by October 1, 2009, your child will be assigned a physical with the school physician.
Annual Screening
Your student’s health is important to their achievement in school. There are health screenings that the school nurse will begin in September in Phys. Ed. classes. All students will have their heights and weights measured. Students new to Lancaster Schools and 10 th grade students receive vision and hearing screening. Please remind your student to wear contacts, or have their glasses with them on a regular basis. This will give us the correct results in our testing, and save you unnecessary visits to your eye doctor. You will be notified in writing if your student has any difficulty with the screenings. If, at anytime during the school year, you feel that your student is having difficulty, please contact the school nurse, Mrs. Murphy.
Policy regarding Hand-Sanitizer Use
The use of Hand Sanitizer has been approved by our Chief Medical Officer for use in our school. He did advise that hand washing with soap and water is best, but he did approve the use of the hand sanitizer for use when soap and water is not readily available.
If any parents/guardians object to the use of hand sanitizer for their students, please send in a note requesting that to the Health Office and we will make faculty/staff aware.
